For Skype video-appointment:

1. You have to have a Skype program on your computer. To download Skype for free on your computer visit the following web site: DOWNLOAD SKYPE  and follow the instructions.

Please send your Skype Account information to us in advance of your appointment.

2. Schedule your appointment. To do that you can call us at 416-357-9561.

3. Please complete the following NEW PATIENT INTAKE FORM and submit it to us electronically prior to your appointment. 

4. Please read and sign PATIENT CONSENT FORM  Then you have an option of scanning and emailing it to us or faxing it to us to the following fax number: (416) 514-0264 with Attention: Olga Cappuccio.

5. Pay for your appointment online via paypal (see below).

6. On the day of your appointment. Please be on time. Make sure it is a Toronto (Ontario, Canada) local time. For information on the current time in Toronto CLICK HERE.
